- health or mental health information e.g. why you want to book an appointment for assessment and/or therapy

- Assess your request for appointment, make contact with you to discuss your referral and book an appointment
When you sign up to the Empowermum’s newsletter, I collect personal information from you, including information about your:
- name
- email address
When you sign up to the Empowermum’s newsletter, you are consenting to be emailed for the purpose of the 7 day challenge of “Get Me More Time” as well as the general Empowermums newsletter. You may unsubscribe at any time.
Providing some information is optional. If you choose not to enter contact details such as a phone number or email address for the contact form, I’m unable to contact you to respond to your inquiry or referral.
I keep your information safe by storing it in an encrypted file and not sharing this with third party services .
You have the right to ask for a copy of any personal information we hold about you, and to ask for it to be corrected if you think it is wrong.
